Below is the text of the "Authorization to Collect Personal Information", a statement that you are required to read, understand, and sign. The home share application will be rejected if you do not agree to, and do not sign, the printed form of this statement.
Once you have read and understand this statement and the "Good Faith Agreement", you can print out both documents on one 8.5"x11" piece of paper (portrait, set to "fit page"), then sign and date where indicated.
Authorization to Collect Personal Information
"By my signature below, I hereby affirm that all of the information I will provide on the application is true and correct to the best of my knowledge, and I hereby authorize the homeowner or his agent, to obtain printed and/or digital copies of my credit report, and/or criminal history report, and/or background report from one or more national credit reporting agencies or online tenant screening sources. I fully understand that thirty-four dollars ($34 per person) of the application fee will be used to cover the cost of obtaining these reports. I also understand that I have the right to dispute the accuracy of any information obtained about me, and to know which sources were used."
