Reading the following information is ONLY necessary IF you are completing and submitting the online application. If you are printing and mailing the paper version of the application, you don't need to continue reading this page.

IF you are filling out the online application form, it is very important that you do not include your social security number (SSN) anywhere on the online form. The online application form is not secure.
The following instructions describe a safe way you can send your SSN (and drivers license number if you so choose) via email IF you are using the online application form instead of printing and mailing it.
Seven Steps to Safely Send Your SSN via eMail
STEP 1 - Open a new message window in your email software.
STEP 2 - On the "Format" menu, turn OFF HTML formatting (aka "Rich Text"). The email should be sent via "Plain Text" - pure ASCII text.
STEP 3 - In ALL lower case, type in the send-to email address shown below in the black rectangle.**
**It is a graphic rather than a link, with different fonts and font sizes to keep it from being read by bots.
STEP 4 - In the subject field, type a descriptive word or sentence.
STEP 5 - In the body of the message type your last name and, without hyphens or spaces - and in ALL CAPS, spell out your social security number (SSN), each part on a separate line (at the hypens, making 3 lines of type). Here's an example (with color added for clarity):

STEP 6 - Type your drivers license number (DLN) below the third line of the SSN if you don't want to submit it on the application form, but do not use any special formatting as you did with the SNN.
STEP 7 - Send the email - Your application will not be accepted without this piece of information.
If this is too much hassle for you, you can send your SSN and State drivers license number (DLN) via email without doing anything special - but do not type it anywhere in the online application.
